Mental Health Outpatient Therapist Position Description

Reports to:  Clinical Director

 Location:  School based/Outpatient Office

 Job Type:  Full Time/Part Time

Position Summary:

The school- based/Outpatient Therapist provides mental health services to children/families/adults in either the outpatient or school setting.  This role involves assessment, diagnostic evaluation, treatment planning, and the delivery of psychotherapy using evidence-based practices.  The therapist works in collaboration with other members of the child’s treatment team to ensure appropriate level of care.

Key Responsibilities:    

  1. Clinical Services:
    • Conducts initial assessments and diagnostic evaluations using standardized tools and clinical interviews with the client and family members.
    • Develops and implements individualized treatment plans based on client’s needs and goals.
    • Provides individual/family sessions utilizing evidenced based therapeutic modalities.
    • Monitors and documents clients’ progress and adjusts treatment plans as necessary.
  1. Collaboration and Consultation:
    • Coordinates care with other members of the child’s treatment team including teachers, parents, case managers and community support associates.
    • Participates in multidisciplinary team meetings to discuss and evaluate treatment needs.
    • Provides consultation and support to families and caregivers as needed.
  1. Documentation and Compliance
    • Maintains accurate and timely clinical records in compliance with state and federal regulations as well as organizational policies and procedures.
    • Ensures documentation of all interactions, treatment plans, and progress notes are entered into the electronic health record within a timely manner, (48 hours for progress notes).
  1. Professional Development:
    • Participates in ongoing professional development activities to stay current with best practices in mental health therapy.
    • Attends regular supervision, staff meetings and training sessions as required.
    • Maintains licensure and certification through continuing education and compliance with state requirements.
  2. Community Outreach and Education:
    • Engages in community outreach activities when appropriate.
    • Provides education and resources to clients, families, and teachers when appropriate.

Qualifications:

Education:  Master’s degree in Social Work, or Counseling from an accredited institution.

Licensure:  Must possess an LPCA, LPCC, CSW, LCSW, or LMFT in the state of Ky.

Experience:  Minimum of two years of clinical experience.

Targeted Case Manager Position Description

Reports to:  TCM Supervisor

 Location:  Outpatient Office/Community Setting

 Job Type:  Full Time/Part Time

Position Summary:

The Targeted Case Manager is responsible for assessing, coordinating, monitoring and evaluating services to clients with a mental health diagnosis and severe emotional disability.  The TCM works closely with clients, families and service providers to ensure clients receive appropriate support to meet their needs.   

 Key Responsibilities:   

  1. Client Assessment and Intake:
    • Conducts comprehensive assessments to determine client’s needs, strengths and resources.   
    • Engages clients and families to establish a trusting and supportive relationship.
    • Develops individualized service plans based on assessment findings.
  1. Care Coordination:
    • Identifies and coordinates appropriate services and resources based on client needs.
    • Facilitates communication between clients, families and service providers.
    • Ensures services are tailored to meet the needs of each client.   
  1. Advocacy:
    • Advocates for clients’ needs and rights within various systems.
    • Assists clients in overcoming barriers to accessing services
  1. Monitoring and Evaluation:
    • Regularly reviews and updates service plans based on clients’ progress and changing needs.
    • Monitors client’s goals and adjusts interventions as necessary.
    • Maintains accurate and timely documentation of all case management activities.
  1. Professional Development:
    • Participates in ongoing professional development activities as needed.
    • Attends regular supervision, staff meetings and training sessions as required.
    • Maintains compliance with state medicaid requirements.

 

 Qualifications:

Education:  Bachelor’s degree in social work, psychology, or human services field.     

Experience:  One year of post graduate experience in  working with children (or) Master’s degree required. 

Must successfully complete Targeted Case Management Training within first 6 months of employment

Community Support Associate Position Description

Reports to:  CSA Supervisor

 Location:  Outpatient Office/Community Setting

 Job Type:  Full Time/Part Time

Position Summary:

The Community Support Associate works with clients who have a mental health diagnosis, to provide social skills training, emotional regulation skills, and behavioral intervention support in community settings.  The CSA works with the client to achieve the ability to function independently within the community.      

 Key Responsibilities:    

  1. Plans and facilitates community-based activities.
    • Engages clients in social activities and promotes social interaction within the community.
    • Provides assistance with daily living skills to increase independent functioning.
    • Provides emotional support and mentoring.

 

  1. Collaborates with treatment team:
    • Participates in service planning meetings and consults with the treatment team to identify the needs and level of support for the client.

 

  1. Completes documentation in a timely manner following Medicaid regulations:
    • Completes notes and care plan accurately, within 48 hours of service provision.
    • Monitors clients’ progress towards goals and adjusts interventions as necessary.

 

  1. Professional Development:
    • Participate in ongoing professional development activities as needed.
    • Attend regular supervision and training sessions as required.
    • Maintain compliance with state requirements.

 

 Qualifications:

Education:  Must be 18 years of age or older and have a High School Diploma or GED certificate.      

Experience:  One year of full-time experience working with individuals who receive services for treatment of a mental health disorder. 

Must successfully complete within 6 months of hire the department led or approved training program for Community Support Associate (Ky State Regulations 908 KAR 2:205)

Must participate in annual training each subsequent year of employment as stated in (Ky. State Regulations 908 KAR 2:205)
